As an aside I am aquianted with two Dr.'s that have told patients , I'm not being your heart Dr if you refuse to quit smoking and another that told an alcoholic, your liver is your own doing, stop drinking for 6 months, if your alive we will talk.
At the ER level you will get seen and treated but going forward not all specialists will waste time on you. Some of them actually want to help people , the abusers dont go to the front of the line.

I don't understand the smell test. I have some organic cartridge masks and they keep out the smell of paint thinners etc but I don't know about covid nano bombs. Mine also have exhale vents. While I'm protected from someone else's garlic breath they aren't protected from mine.
FWIW the organic mask also filters out the smell from fecal matter and vomit if they tend to make you gag.
I was wearing an N95 the other day but it was only for my protection from some very fine sawdust. It too has an exhale vent so not OK for public places.
My stock of unvented KN-95 masks has the usual exhale issues, fogging glasses etc. That would indicate some degree of exhale spread. Keep in mind this is an odds game. Anything that reduces transmission reduces the odds of contracting the disease. The statistics indicate that what we are doing makes a big difference.
And as mentioned a refinery won't let me into an operating area if I have stubble, the concern being H2S gas and mask sealing.
N95 fit tests are for your protection. The masks were never designed to protect others. I have never done a fit test and have no idea what the smell is. I suspect it could be some chemical with a specific particle size. We all know that certain smells will pass straight through an N95.

Some of those are really easy to enforce as well. For anything requiring registration (trail permit or sport), the premium could be collected and remitted as part of the registration. For things like booze or smokes, add the premium to the purchase. Obese is hard. When you see a doctor they weigh you and you get a BMI bill? Many people will avoid doctors. BMI calc when you renew your drivers license? Rock climbers/skateboarders are hard to enforce as they are often informal activities. What's the difference between a rock climber and person out for a walk to get exercise in a slip and fall injury? I'm not challenging your list. Politicians are money grubbing idiots and if they think they can suck a buck out of people, they will do it. I am sure their will be an exemption for elected members of government in whatever they pass. The vast majority would fail a BMI check (with a few exceptions like the MP that kept showing his &^%^% while parliament was in session).

I like the idea. We already tax smokers, tokers, & drinkers -- why not the anti-vaxed?
I don't think it's the homeless or street dweller that's the big problem -- give them a pass. It's the able bodied folks that are clogging ERs that should be taxed. Quebec just announced they are doing it, I believe the taxing starts next week and it's been promised to be hefty.
As for the stupid, I'm patient and tolerant with stupidity -- but not with those who are proud of it.

About 18 months ago my wife was admitted to Freeport hospital for assessment regarding her brain injury. The first thing they did was take away her walker and put her in a wheelchair because she was at risk to a fall. After 4 weeks one morning she tried to get from bed to bathroom by herself. She slipped and fell. Knocked out her front tooth and broke a molar, opened a gash in her head.
A doctor had to be called in and supplies scrounged to stitch her up. While the doctor was stitching her up, he told one nurse where to shine the flashlight and the other nurse (foreign in training) how to cut the suture end. After 3 stitches, she fainted. Never seen blood before.
Oh, the doctor asked why she wasn't wearing a purple (risk to fall) armband.
You can't make this stuff up.
The next few weeks is gonna be wild.

We have lost dozens ( tens of dozens) of nurses in the pandemic with over work , under staffed , under compensation and quite shiity conditions and our answer is import , yet to be CDN certified talent . Um oky……

The further a person is from the problem the more they minimalize it. When it's them, their kid, spouse, lifestyle it's suddenly an issue. Until then we want to hear about lower taxes.
We had to do respirator fit tests when I owned part of a composites (fibreglass) shop. The smell was sickly sweet. If you could smell/taste it the mask didn't fit.

Well then I guess we don't have to worry about taxing the unvaxxed then if that's the reasoning to do it

That styrene smell when you walk past anybody doing resin layups . Did you just do polyester and vinylesters ?
I’m using some epoxies right now , if you follow all the safety guidelines you gown, glove , mask , ventilate the space and roll it out with a 12ft extension pole , preferably through a window in another building.

They aren't though. Overall only 20% of people in the ICU are due to COVID, the rest are there for the usual reasons. Our healthcare system is so pathetic that it can't handle moderate increases in ICU bed occupancy. This was happening before the pandemic as well: 1, 2. The reality is that the government has been living beyond its means for quite awhile now and COVID has really exposed that.
Taxing the unvaccinated is a bad idea that only plays into the talking points of antivaxxers and their ilk. We don't tax people extra for having kids and sending them to public school, just like we don't give people tax rebates for not having kids and not sending them to public schools. Everyone pays into the system so they can use it as they need it.
Besides, you really do not want to invite the government into your life like this, because it will open the door to taxing every little unhealthy thing. I don't want to start paying a tax because I ride a motorcycle, or other nanny state type taxes on fast food, sugary drinks, etc. It's especially hypocritical when the government has a monopoly on selling literal poison (alcohol), which probably causes damage that will never be recouped by any amount of sin taxes.
