Requirements for collision reporting?

Hey guys so Thursday was unlucky for me as some lady reared into my bike REAL hard. The damage to replace my fairings and aftermarket stuff well exceeds $2000.

Now she was nice enough to leave me her name and number. Can someone tell me exactly what's required to report it at the collision center.

She let me talk to her business adviser as the car was under the business name; So far he gave me the Insurance Name and Policy number; I asked him about the plate # and the license # of the lady and he has yet to get back to me.

If in case he doesn't want to provide the address, what are the minimal requirements to report it?

Thanks

*Edit* The bike doesn't start by the way, and the damage looks serious as the bike doesn't even start, i've bypassed the tip over sensor and still no luck. I had to tow it to my house...also last night the only way i was able to start it was by a jump start.. and then guess what..I think my gear box is messed up..it's stuck in gear 1 and i cant even kick unto gear 3.. it's completely stuck. When i turn off the bike and try to start it again, same issue the only way is to start it is...jump start..

Another thing i noticed is that even if the clutch is disengaged.. the bike roles at a stead speed of 7km/hr...IN NEUTRAL!!

Name, drivers license #, license plate #, insurance company and policy #.

I'd be reporting this post haste!

You may need to have it taken to a dealership or shop to get an accepted damage estimate. Check with your insurance co. Reporting to them with the other driver's ins. info is more important than the collision ctr. If the fault is the other party, you are covered even if you only have liability on your bike. No fault ins. I've had a bike written off for being knocked over. Just the cost of bodywork can be enough to have a write off.

Yes, I think this will be a write off as well, the cost to replace the parts is a grand total of $3149.18. And this doesn't even include the labor & figuring it what is wrong with it i.e not starting as everything inside the bike is less then 1 year old.. stator, rectifier, battery, etc..

You only need to report only if the total amount of damage is over $1000 (I don't recall if that's both parties or just yourself) I would report it right away, once you have reported it - the lady who hit you will have 48hrs to report or things will go real bad for her. Make sure, you have told her "rep" that this has been reported to the collision reporting centre this should make them move their ***. Your insurance should pretty much take care of the rest (if you haven't already spoken to them).

Another rear end accident??!

Hope you are ok. You should definitely file the police report just in case at the collision centre.

It is $1,000 damage total between all vehicles.

^^ Exactly.

If the vehicles are NOT driveable, the Police should always be called. AND DON'T DIAL 911 Unless someone is injured or a vehicle is on fire.

For anyone else reading this, always be sure to get the other parties:
-License Plate number
-Drivers License Number
-Insurance company name
-Insurance company Policy number

If your vehicle is driveable, quickly make note of where it happened and what happened while it's fresh in your mind.

....and if your vehicle is drivable promptly tell all tow drivers that attend to **** off and don't let them try and trick you into thinking you vehicle isn't drivable cause a headlight is broken or you're missing a mirror.
bloody vultures.
