For a first attempt, I would just knockoff something like a Briggs or low cc honda. Piggyback on their thousands of hours of engineering and expensive lessons. That also lets you buy parts like rings and valve springs that would be painful to make.

If I wanted to build a tiny engine, I want multi-cylinder. They can rev to the moon and sound cool. The number of hours and cost would make it a very expensive toy.
