Air India did just fire a trainer back in March for failing to do his job. So have there been some lapses in procedures and led to complacency somewhere? Who knows.

One thing is for sure, the plane didn’t pushback, taxi, and take off with the engine switches in the cutoff position regardless of what ultimately happened.

Exactly. The plane took off after the checklist was completed. And then.......

My understanding of that aircraft the fuel supply is literally a toggle switch , except to return them to off you need to lift the toggle then pull back , so you can’t catch a sleeve or hit it accidentally. Like a shift gate on your manual car you can’t go to reverse without a pull up or push down. Muscle memory excuse would only work if you’re an idiot. Those switches got switched off .
