Steen Hanson has Ohlins fork mounts listed from 38mm-55mm, 41mm is part #96-2235-04 with a listed retail of $114.95

Personally for that price Id machine my own....
 
When I was running a Ohlins side mount on my SRAD with a 636 swap ....I paid around $42-$45 for the fork clamp.
 
They are $20 all day long on ebay...And any Harley shop can and will have them on the shelf. Harley guys use them in 41mm to mount extra lights to their baggers and softails etc.

My YZF600R racebike has 41mm forks so I got one from a Harley shop online. It was $23 and change shipped. f you can find one, a 2 pc design will be easier to install as you won't need to remove slip ons and top triple clamp.
