Yep thanks everyone. It hasn't happened since I posted this, although I haven't done very many warm starts either (it has only ever happened on a warm start so far). I will keep an eye on it and if I hear it again I'll get in there and lube it. Cheers

Close, but not quite. They were clearly separated, and the two lights doubled as signal lights as well. This has separate signals. Although it could have been a mod because I don't think that's actually legal.

Highly possible, I feel fairly confident with my front brake control, but I really don't pay much attention to the rear brake when braking hard. I honestly can't say if I press it hard or soft, my guess would be I jam it.
