If its BRAND new, you probably wont need the UVIP (used vehicle information package), although you will need some sort of bill of sale. Just a letter from the seller saying that they sold it to you, include your names, date, VIN, make and model of the bike etc, and for how much you bought the bike for.
If the seller has the bike registered in their name you get them to sign over the ownership part of the permit. Since is not registered, the bill of sale should be enough I think. Then take all that to the MTO. You'll have to pay the PST on the sale amount and they'll issue a new ownership. If you want to get it plated you'll need to get insurance and get the bike safetied as well.
